In a review of program emergency drills, it was observed that no fire drill was documented for November 2024.
Required correction: Emergency drills, lockdown and active shooter on premesis drills must be held at least quarterly but often enought so that occupants are familiar with the drill procedure and their conduct during a drill is a matter of established routine. Fire drills must be held monthly and be consistent with local fire department procedures. A record of all emergency drills held over the last 12 months must be maintained by the facility or center including the date and time of drill, number of adults and children participating, and the amount of time taken to evacuate. Correct immediately. Provide written statement of process program will use to ensure drills are conducted and documented according to required timeline and regulations. Correct by 12/18/2024

Small broken crayons and foam stickers on shelf near the door accessible to children under the age of three in the Toddler room 146.
Required correction: Any materials accessible to children under three years of age must not be a choke hazard or able to be inhaled. Correct Immediately. Assistant Director corrected at time of visit by moving the items to a high shelf inaccessible to children. No further action needed at this time. Correct by 01/29/2024
On the outdoor playground (Toddler and Preschool) sections of the planter wood had warped and left a large gap with sharp edges exposed.
