This is a direct violation of stipulation K on the settlement agreement which states, "Staff members must complete any trainings required for their assigned position prior to being placed in the classroom. This may include, but is not limited to: training and orientation in facility policies and procedures, standard precautions, CPR and first aid, safe sleep, shaken baby/abusive head trauma prevention, orientation for a toddler program staff aide and medication administration. Verification of completed training must be available for review at all times."

At least two children from classroom 112 playing on the ECE playground equipment measuring approximately 30 inches in height with frozen resilient material (woodchips) under the equipment.
Required correction: Immediately make all hazardous areas inaccessible to children. Submit plan of action with written response to specialist. CORRECTED AT TIME INSPECTION - staff members had all children cease playing on the equipment and moved to other areas of the playground. Correct by 01/05/2024
