At the time of visit, observed the facility to be lacking documentation of the times children arrive and the times children leave each day. Also observed, in classroom Frogs the teachers stated they had 16 children in care but the attendance tracker showed they had only 15 in care.

Required correction: Immediately, the time that children arrive and leave must be recorded daily and staff must know the number of children in their care at all times. Submit a written statement of compliance by 1/22/2025. Correct by 01/15/2025

At the time of visit, In Bear 3A classroom observed cleaning supplies labeled "keep out of reach of children" in a unlocked drawer accessible to children.

At the time of visit, observed diaper changing supplies on a brown shelf in the bathroom accessible to children.

Required correction: Immediately, all diaper change areas must have a place inaccessible to children for storing all diaper change supplies and products. Corrected at time of visit, the classroom teachers moved the diapers supplies up and out of reach of children. No response is needed. Correct by 01/15/2025
