Required correction: Abuse and neglect inquiry background checks must be renewed by resubmitting an inquiry form and current fee to the department for processing. An updated clearance letter must be obtained before 5 years from the date refected on the current clearance letter. ***All individuals requiring a background check must review the "Navigating the BIU's Webpage" powerpoint within 24 hours. Send verification that SM1 reviewed powerpoint and send documentation of TRAILS clearance letter with written response to specialist. Correct by 09/23/2025

Reviewed daily name-to-face attendance logs for 11/8/23 and identified written attendance verification during transitions was not documented accurately in the Wraparound classroom. Observed 8 children in care, however 20 children were still noted on the name-to-face log.
