Approximately a dozen small white clothespins affixed to Middle Room walls, small enough to fit in a choke tube, and in an area of facility accessible to children less than 3 years of age, posing potential choking hazard.
Required correction: Immediately ensure toys, toy parts, furnishings, equipment and any materials accessible to children less than 3 years of age do not pose a choke or inhalation hazard. Corrected at the time of visit. Staff removed clothespins from walls in Middle Room and made them inaccessible to children less than 3 years of age. NO RESPONSE REQUIRED. Correct by 10/21/2025
