On the toddler outdoor patio, observed a portable slide and a pikler triangle approximately 24 inches tall placed on gym mats. Documentation that mats meet current federal safety standards were not available for review. On the middle toddler playground, observed approximately 1 inch of loose pea gravel in front of the permanent climber's slide. Beneath the loose pea gravel was a compacted mix of pea gravel and dirt. On the preschool playground, observed approximately 2 inches of loose pea gravel in front of the permanent climber's slide. Beneath the loose pea gravel was a compacted mix of pea gravel and dirt.
Required correction: Ensure that all climbing equipment 18 inches or higher has resilient surfacing of at least 6 inches in the use zone surrounding the equipment and the resilient surfacing is Department-approved as described in 7.702.72.C.1.b. **Partially corrected at the time of visit.
