Gap between the bench and the platform on the pirate ship that measures approximately 8.5” and several gaps that measure approximately 7" in wooden structure in the middle of the playground posing potential entrapment hazard.

Required correction: Immediately ensure all pieces of playground equipment are designed to guard against entrapment and strangulation. Any openings in gross motor equipment above ground must be smaller than three and one half (3 ½) inches or greater than nine (9) inches to prevent entrapment. Document correction in written response.
