At the time of visit, in Room 407, two children went into the girls bathroom in the hallway outside of the classroom alone while a teacher stood in the doorway of the classroom. The children were not in line of sight.
Required correction: Immediately, all children must be under direct supervision at all times. Submit a written statement of compliance by 11/10/2024. Correct by 10/10/2024

At the time of visit, in Room 407, staff transitioned children from the classroom to the cafeteria for lunch without recording the transition or bringing an attendance verification tool with them to the new location.

In room 407, several sharpie markers labeled 'keep out of reach of children' were in a plastic caddy behind the teacher's desk and were accessible to children.
Required correction: Immediately, all items labeled 'keep out of reach of children' must be inaccessible to children at all times. Corrected at time of visit, the co-directors moved the sharpie markers to locked cabinets in both rooms bringing the center into compliance. No response needed. Correct by 10/10/2024

At the time of visit, in the girls bathroom, a roll of loose trash bags were on top of the paper towel dispenser and were accessible to children.
Required correction: Immediately, all loose plastic bags must be inaccessible to children at all times. Corrected at time of visit, the center director moved the bags to a high cabinet bringing the center into compliance. No response needed. Correct by 10/10/2024

At the time of visit, in room 407, a pair of adult scissors was in a plastic caddy behind the teacher's desk, and a staple remover was on top of a filing cabinet behind the teacher's desk and both were accessible to children.
Required correction: Immediately, all sharp tools must be inaccessible to children at all times. Corrected at time of visit, the center director moved the items to a locked cabinet making them inaccessible to children. No reponse needed. Correct by 10/10/2024

At the time of visit, in room 404, a stack of chairs was near the art drying rack and was a safety hazard for children.
Required correction: Immediately, all indoor furnishings must be sturdy, safe, and free of hazards at all times. Corrected at time of visit, the center director unstacked the chairs bringing the center into compliance. No response needed. Correct by 10/10/2024
