Identified through staff statements from SM1, SM2, SM3 that staff will not offered an activity until after at least 40 min to children that have not fallen asleep they will sometimes move the ch to be near staff while they work on curriculum or observeation notes.

Required correction: Children who do not sleep after 30 min must be allowed to move to another area and provided with quiet toys and equipment to play with. Correct immediately and send written reponse to specialist. Correct by 09/23/2025

Tiger class SM1 stated that she was in a corner with ch1 patting them when she observed ch2 move towards door and she had no visual on the ch2 or ch3 near the door. Day of visit observed SM2 also sitting in same corner also having no direct visual of ch3 in hall near door. Observed in Lions room also a a ch4 placed behind staff rocking chair so SM 4 had no visual of Ch4 in the corner.

Required correction: All children must be under the direct supervision at all times of a qualified adult who has been assigned the responsibility to supervise. Correct immediately and send written response to specialist. Correct by 09/23/2025

Submit verification that staff members have reviewed the powerpoint within 24 hours. Submit documentation of Out of State background check, for staff members with written response to the Specialist.
