2.209.A.8 · Correction due July 21, 2025

Identified that staff went to Northglenn Rec Center pool and observed that the field trip authorizations to be for Kiwanis Park & Pool. Identified that 18 children went on field trip and observed only 14 authorization received. Identified by staff statement that Kiwanis pool was closed for inclement weather. Program went to Northglenn Rec Center instead. Identified that CH that was left was missing field trip authorization.

Required correction: Obtain field trip authorization for all children who participate in field trips. Correct by 07/21/2025

2.223.A.1 · Correction due July 21, 2025

Identified by staff statement Jo received phone call that other bus had left CH at pool. Jo's bus was still in the north parking lot so she went to pick up CH. Identified by staff statement that CH was left for approximately 10 minutes. Identified by staff statement that CH was in cabanna changing and program left with out child.

Required correction: Maintain direct supervision at all times of a qualified adult who has been assigned the responsibility to supervise. Correct by 07/21/2025

Identified by staff statement CH1 was left behind at Northglenn Rec Center Pool for approximately 10 minutes. Identified that CH1 was in dressing room-cabana when program left. Identified incident occurred on July 17 and no report has been made to the department to date.

Required correction: Submit no longer than 24 hours a written report about any child who has been separated from the group outside of the supervision of their assigned staff member. Correct by 07/21/2025

Identified by staff statement that group separated one group with lead with 8 children and 1 group with 11 with aide. Identified by Savannah's statement she did not do a name to face check with children and simply counted them. Identified that Nancy clarified that they had 10 children and Savannah stated yes. Identified that Savannah should have had 11 children with her. Identified by Savannah's statement that she miscounted the children. Identified from Savannah's statement she did not have an attendance rooster with her.

Identified by staff statement that parent of CH parent was not notified of lost child incident. Identified that policies and procedures state "In the event a child is lost or separated from the group every effort will be made to locate them, The police department will be contacted for assistance and the parents will be contacted immediately.
