Observed the facility did not have the correct information regarding the procedures for filling a complaint posted, there was no address and the telephone number was not correct. Previosly cited on 7/23/2024.

Observed in Step 3 and step 6 Classrooms soiled rugs with dark spots that appeared to be dirt marks and the children were playing on them. Additionally on step 3 classroom observed a large tumble mat that had three (3) small pieces of tape on it , the tape had been pulled out halfway posing a potential choking hazard.

Required correction: Indoor and outdoor equipment materials and furnishing must be study safe and free of hazards. Correct immediately. Send verification with written response to specialist. The facility's Director corrected the violation at the time of the inspection by removing the rugs from Step 3 and step 6 classrooms and also by removing the large tumble mat from Step 3 classroom. No response is necessary. Correct by 07/18/2025

The classroom Teacher correct the violation at the time of the inspection by moving the small part of a brittle game to a place where the children could not access it. No response is necessary.
