Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On July 24, 2026 in Space FH, a three (3) year old child was pulled to a cot by one arm during rest time.

Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. One (1) set of infant bottles was not dated.

Equipment and furnishings were not sturdy, stable and free of hazards. A wood board was loose under the ramp leading to the playground. A screw was poking out of the board. A nail was exposed on the stairs leading to the playground.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A magic eraser was stored on shelf accessible to children. The storage closet in the kitchen was unlocked. The closet had cleaning supplies stored inside and accessible to children.
