Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu for the week was not current for the week.

Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. In space F there was an electic fan on the floor, uncovered and in reach of children. It was not in operation.

Prior to employment a Criminal Background Check was not completed. Staff member B.B. initiated a criminal background check before she began work, but it was not completed nor did she receive a qualifying letter before she began working with children.

Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The larger playground’s equipment was in good repair but the maintenance staff was in the middle of upgrading the bottom of the fence to be in compliance. While the depth of the bottom of the fence has openings that are not less than three and one half (3 and ½) inches or greater than nine (9) inches There were also protrusions with rugged and pointed end that could be a hazard. While most of the playground is finished and in compliance, there is an area near the gate which is out of compliance.
