The EPR Plan did not include the date of the last revision of the plan. The staff had completed a review of the EPR plan but the actual plan was not updated from the previous year.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. In the classroom serving four (4) and five (5) year old's a child's medication recently expired and while the facility has reached out to the parents to bring in a non-expired medication, the parents have not.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the main hallway there was a closet that houses cleaning supplies and it was unlocked.
