Children were not adequately supervised at all times. One (1) child walked across the hallway unaccompanied to use the restroom in Space F15. The teacher was unaware the child left the classroom.

Outdoor play equipment was not age and developmentally appropriate. New playground equipment installed in the Spring of 2023 had a climbing structure for children ages 5 -12 years of age and a set of swings for children 5-12 years of age. Children including four year olds were observed playing on the equipment today.
