Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection completed on 10/24/25 was received on 11/4/25.

The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The EC/NC Pre-K Specialist did not have documentation of reviewing the facility's safe emergency medical care plan.

Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. An opened bag of potting soil was stored on the floor and a tube a E6000 was stored on the counter, accessible to children.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. An emergency medication expired on 10/19/24 according to the pharmacy label.
