Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. During today’s visit the provider’s personnel and training files were reviewed. It was observed that the provider that the provider was due to successfully complete the two year renewal of the required First Aid certification prior to June 30th, 2026 but this did not occur until July 18, 2026.

Items used for starting fires, such as matches, accelerants, and lighters, were not kept in locked storage. During today’s visit while transitioning from the kitchen to the living room of the home a candle was observed lit and being stored on the counter space near the kitchen sink.
