Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The railing that leads out to the outdoor play area had areas in the corner by the door and at the end of the ramp that had spaces greater than 3 1/2 inches between railing spindles.

The disinfectant and sanitizer spray was located on top of a shelf that housed children activity materials and was not at least five feet from the floor. Sanitizing wipes and all purpose spray were located on the kitchen counter between the classroom and kitchen and were not stored in locked storage.
