Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space #1 and #3, there was a bottle of Benadryl stored in a clear bag that was not in locked storage.

Equipment and furnishings were not sturdy, stable and free of hazards. The wood boarder at the base of top of the slide and boarders around the garden beds had paint that was chipping.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. An infant in space #1 was asleep. It was documented that the infant was placed in the crib at 9:15AM however, a visual check was not completed until 9:37AM while I was in the classroom.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. One Benadryl on site, for a child with a medical condition requiring the medication when needed, does not have written permission from his physician. The child is 4 years old and the Benadryl instructions states 2-5 years do not use unless directed by a doctor.
