Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident report was not on file for an incident that occurred on May 22, 2026.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. Based on interviews a staff member tap a child on the hand because he spilt his water. The same staff member pulled the same child by 1 hand from under the table and stated "he shouldn't have his ass under the table". The same staff member popped several children on the hand because they wouldn't stay on the carpet, crawled under the table, and wouldn't sit still.
