All equipment and furnishings were not in good repair. There were deteriorating wood pieces and/or sections on the wooden stationary structures on the playgrounds designated for two-year-old children, three and four-year-old children, and school-age children. On the playground designated for school-age children, there was a torn section of the artificial surfacing approximately six inches square under the bar structure. On all the playgrounds, the metal poles and some of the metal equipment pieces had chipping paint.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space 4 a can of Equate aerosol sunscreen was stored in an unlocked cabinet. In Space 2, there were two batteries laying on the counter near the sink.
