Children were not adequately supervised at all times. Upon entering the center, as we walked directly into space #2, we observed six children sleeping with no caregiver in the room, one caregiver was in space #3 (the caregiver was getting something) and the other caregiver was in space #1.

Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The fence had some gaps at the bottom of the fence in some areas that measured between six inches to eleven inches.

Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence near the parking lot/ near the bamboo measured five feet eleven inches with protrusions.
