Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The posted menu was dated June 22, 2026, to June 26, 2026.

All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The stationary bus and spring rider structure had zero mulch depth under and around the play structure, and footings which anchor the equipment were exposed.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In the girl’s bathroom, was one (1) plastic package containing Breeze Feminine Maxipads stored under the unlocked cabinet below the handwashing sink and accessible to two (2) children two (2) years of age.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the girl’s bathroom, adjacent to Space #3, was two (2) electrical outlets that were not covered with a safety plug.
