Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Fire inspection was not completed, despite emails sent to the fire inspectors.

Equipment and furnishings were not sturdy, stable and free of hazards. Blocks in classroom 3, were splintered. shelf in class 4 had exposed nails. several shelves in each classroom are unstable (black crates) and one shelf has chips and torn sidings.
