Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last approved fire inspection was received July 2, 2025.

Children were not adequately supervised at all times. On the playground three (3) teachers were observed sitting at a picnic table talking and looking at their phones. Children were running around the playground playing and several times went around the corner out of teachers sight of the teachers. The teachers were not supervising the children.
