All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Space 208, Clorox bleach, Clorox wipes, and aerosol spray were stored in an unlocked cabinet. Space 209, Clorox bleach, Clorox wipes, and aerosol spray were stored in an unlocked cabinet. Space 219, Clorox bleach and Lysol wipes were stored in a storage cabinet which was unlocked on one side. Space 222, one (1) Lysol aerosol can, one (1) Lysol non-aerosol spray, and six (6) Clorox wipes stored in an unlocked cabinet above five (5) feet.

Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children. Space 222, One (1) laminator was observed not in use sitting on top of the built in storage below five (5) feet.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Left over medication was stored in a classroom. The medication was for a child that no longer attends the program.

Children were not adequately supervised at all times. Staff members failed to adequately supervise an 8 year old child, when the child exited the school building, traveled through a parking lot and a field, and was located at another nearby school.
