All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Space 208, Clorox bleach, Clorox wipes, and aerosol spray were stored in an unlocked cabinet. Space 209, Clorox bleach, Clorox wipes, and aerosol spray were stored in an unlocked cabinet. Space 219, Clorox bleach and Lysol wipes were stored in a storage cabinet which was unlocked on one side. Space 222, one (1) Lysol aerosol can, one (1) Lysol non-aerosol spray, and six (6) Clorox wipes stored in an unlocked cabinet above five (5) feet.

All electrical appliances were not used in accordance with the manufacturers instruction. Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children. Space 222, One (1) laminator was observed not in use sitting on top of the built in storage below five (5) feet.

Prior to employment a Criminal Background Check was not completed. Staff member D. Mazzola hired 7/29/2026 Criminal Background Check was not completed prior to employment. Staff members qualifying letter was dated 8/12/2026.

Left over medication was stored in a classroom. The medication was for a child that no longer attends the program.
