In Space 6, flashlights with easily accessible batteries were stored on a counter lower than five (5) feet.

Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The annual fire inspection was completed July 28, 2025 however was not submitted to the Division within one (1) week of the inspection.

One new staff member's hire date was July 1, 2024, however the medical report was dated July 12, 2023.
