All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. In space D1, there is peeling paint on a wall.

Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. In space A2, there was a teacher's iced coffee drink on the counter.

All equipment and furnishings were not in good repair. The fence between the preschool and older children's outdoor learning environment had a hole with protruding wires with sharp edges large enough for a child to crawl through. The fence gate between the ones (1) and twos (2) outdoor learning environments had gaps with sharp edges in the bottom of the gate that need to be addressed.
