A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child did not have a signed receipt of the NC Summary of the Law on file.

After review of a video, on July 15, 2026, a lead teacher was monitored staring at a child from across the room who was playing with a toy with a stern face, while walking towards the three-year-old child with both hands on their hips. The staff member did not verbalize anything to the child before handling the child roughly.

A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. After reviewing a video, on July 15, 2026, the following was monitored. In space #5, a three-year-old child was awake and playing quietly with a toy that was stored on top of a storage unit that was adjacent to the child’s mat. The lead teacher was monitored, grabbing the child’s mid left arm, with one hand, then lifting the child across part of the mat, pulling the child towards them, then forcefully pushing the child down on the mat. The child was then observed on their knees and elbows. The lead teacher then grabbed and yanked both ankles of the child downward, to position the child in the middle of the mat. The lead teacher then grabbed the left middle arm and thigh of the child and flipped the child onto their side position. The child cried out.

Discipline was related to food, rest or toileting. After reviewing a video, on July 15, 2026, a three-year-old child was forcefully disciplined by their lead teacher due to playing with a toy instead of resting on their mat.

Children were not adequately supervised at all times. On October 20, 2025, the center administrator self-reported an incident of inappropriate interactions between two (2) children in space #3 during naptime. The inappropriate interactions occurred on and off for nine (9) minutes and the adult responsible for adequate supervision was not aware of the children's activities nor positioned herself in the classroom to maximize her ability to visually supervise children during nap time.

When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. Children were seen watching television after naptime for approximately ten (10) minutes without documentation of it on a log or linked to any developmental domain.

Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Five staff did not have documentation showing they completed the health and safety training within 5 years of last completion. Of the five, four didn't complete recognizing and responding within last 5 years of completion and the fifth didn't have proof of completing any of the topics within 5 years of last completion.
