All equipment and furnishings were not in good repair. On the outdoor learning environment, I observed the stairs on the play structure to the far right not installed properly/ not attached.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the kitchen, I observed a container of bleach underneath the kitchen sink. In space #1 and #3, I observed tubes of paint that says "keep out of reach of young children" on the shelves, accessible to children.

The EMC plan was not reviewed with all staff annually and whenever the plan was revised. One out of four staff member did not have documentation of reviewing the EMC plan annually. (GH)

Electrical cords were accessible to infants and toddlers. In space #3, I observed an AC unit inside the window with a cord plugged in to the electrical outlet and accessible to a toddler.

Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The center did not have documentation of a monthly playground inspection for August 2023.
