Number of children exceeded number allowed. On Monday, May 4, 2026, the arrival and departure log indicated there were six (6) preschoolers present from 3:30 pm - 4:40 pm.

All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. The padlock on the shed located next to the play equipment was not locked. Hazardous products were stored inside the shed.
