Children were not adequately supervised at all times. Three children that were in the hall outside their classroom unsupervised. Teacher did not know the students were playing in the hall.

Staff/child ratios applicable to a classroom, were not posted in each classroom. In two classrooms the staff/child ratio sheet did not have the correct age group.

Parent's medication authorization did not include required information. One medication form was expired and others did not have the correct amount of cream to apply to child.

Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Two emergency forms for children did not have a photograph of student on form.

Center did not maintain a record of daily attendance. In the classroom for the 108 the attendance sheet noted there were eleven in the classroom and twelve children marked present on the attendance sheet.
