Equipment and toys were not in good repair and developmentally appropriate. During the walk through it was observed that there were five (5) books accessible in the child care space with torn spines and ripped covers. I

The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The facility’s Emergency Response and Preparedness Plan was reviewed, and it was observed that it did not contain all updated information as required.
