Children under one year of age were not kept separate from children two years and older. Two (2) two year old children were being cared for in Space 1 with a child under 12 months of age. Repeat violation

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. The stucco/cement on the side of the foundation was observed peeling and coming off in the infant/toddler playground posing a choking hazard.
