Enhanced staff/child ratios and group sizes were not met. There were 13 children present in space two. Teacher assisted me with the walk-through, and later a teacher walked into the office while I was typing. Ratio was 1 to 13 and should have been 1 to 12.

Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu posted read that Pineaples would be served, children were served apples.

Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. Children's food is delivered, however the refrigerator where the milk is stored did not have a thermometer to measure the tempreature.

The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. The playground had several empty bags of chips, and other debris scatteredd in several places.

New staff members M.J. and J.B. did have proof showing review and signed acknowledgement of the Shaken Baby Syndrome and Abusive Head Trauma policy but the date reviewed and signed was after caring for children ages zero (0) to five (5).

Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. New staff members M.J. and J.B. medical report, proof of tuberculosis and health questionnaire were kept in the staff file and not in a separate file.
