Children were not adequately supervised at all times. On May 11, 2026 and May 12, 2026, five children ages three to five years of age were left unsupervised. The amount of time the children were left alone is unknown.

A child was yelled at, shamed, humiliated, frightened, threatened or bullied. A preschool teacher was observed by staff members and parents on several occasions yelling at a group of preschool age children.

A violation was found for which there is no item number. In child care centers, toilet tissue paper shall be provided in each toilet room and stored in a clean, dry place. The toilet room shall include or be adjacent to a handwash lavatory. Storage in toilet rooms shall be limited to toileting and diapering supplies. All toilet fixtures shall be kept clean and in good repair. Toilet fixtures shall be child-sized, adult-sized toilets that are adapted to accommodate children, or potty chairs. A large plastic container of manipulatives was stored in the restroom adjacent to classroom #1A. This is a violation of a requirement in 15A NCAC 18A .2817. The violation was corrected during the visit.

Purses and other personal effects belonging to child care center employees shall be kept inaccessible to children and shall be stored in accordance with this Rule, as applicable. In space #1A, an employee's purse was stored in a unlocked cabinet that was less than five feet above the floor. This is a violation of a requirement in .2820(g).

A current activity plan was not posted for each group of children for reference. In Space 1B a current lesson plan was not posted. In Space 1A an outdated lesson plan was posted.

Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. In Space 1A an unlabeled and undated sippy cup of water was in the outdoor bag.

Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. In Space 1A a refrigerator had two thermometers reading 50 degrees and 48 degrees.

All equipment and furnishings were not in good repair. Several riding toy were faded and dirty on the infant and the toddler playgrounds.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In Space 7, one electrical outlet did not have a safety plug.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. in Space 1A dish soap was accessible by children in an unlocked cabinet under the sink.

The center did not have developmentally appropriate equipment and materials accessible daily. Space 1B serving two and three year old children had glitter and googly eyes to use for art activities, both items state do not use with children under 3 years of age.

Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. Fencing between play areas is 4 feet and is a chain link fence with the top rail lower than the top of the chain link fencing, leaving a protrusion.

Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. One Epi Pen observed in Space 1B did not have the pharmacy labeled container on site.

Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. One child's file reviewed today, has a signature page stating the parent reviewed and received the center's discipline policy, however the signature page does not list the child's enrollment date.

A violation was found for which there is no item number. Child Care Rule 0803(11) was not meeting compliance today. A child's current medical action plan states to provide Benadryl for mild symptoms. You stated the parent said you didn't need that medication and took that medication home, but you did not get anything in writing from the parent stating it was not to be administered on site.
