Children were not adequately supervised at all times. A preschool child was strapped into a sleeping chair, and one staff was seated at a table eating lunch and out of sight of the two children who were present. The other staff member present was responsible for supervision of the children and was observed on a cell phone. The child was awake but without any materials in front of them in space a3.
