Children were not adequately supervised at all times. One staff was not aware of two children's activities inside of a play structure which resulted in an injury.

Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident report for an injury which occurred on May 20, 2026 was not on file at the facility.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A prescription medication remained in the classroom after the 14 day course of treatment and was not returned to the family.

A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. Twice on the same day a child was picked up by their forearm(s) and moved from a chair at the table to another part of the classroom. During the first incident, the child was forcibly picked up by both forearms, the child's feet were not touching the floor, the teacher carried the child in this position across the classroom and the child was placed in a cubby. The child was visibly upset. During the second incident the child was forcibly picked up by one forearm and taken to their bed.

A safe indoor and outdoor environment was not provided for the children. In Spaces 4a and 4b diaper creams were stored in the cabinet under the changing table and secured with a child proof lock. A child proof lock does not qualify as lock and key.

A safe indoor and outdoor environment was not provided for the children. A staff members' failure to ensure the child was a safe distance away from the door when it closed created an unsafe environment and a two-year-old child's finger was closed in the door jam.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. In Spaces 5 and 14 permission to administer forms for diaper creams were missing dosage amounts, the name of the medication and a parent signature.

Children were not adequately supervised at all times. On January 30, 2024, a staff member in the classroom for three-year-old children, left the children unsupervised when the staff member stepped out into the hallway, allowing the door to close.

A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In Space 1 there was no permission to administer form. In Spaces 2 and 6 the amount was not specific on the permission to administer form.
