Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A prescription medication remained in the classroom after the 14 day course of treatment and was not returned to the family.

A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. Twice on the same day a child was picked up by their forearm(s) and moved from a chair at the table to another part of the classroom. During the first incident, the child was forcibly picked up by both forearms, the child's feet were not touching the floor, the teacher carried the child in this position across the classroom and the child was placed in a cubby. The child was visibly upset. During the second incident the child was forcibly picked up by one forearm and taken to their bed.
