Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. I observed some children without departure times on daily attendance.

Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. I observed the cabinet underneath the bathroom sink not lock, this is where hazardous products/ creams are stored/accessible to children.

Electrical outlets not in use were not covered. I observed electrical outlets in the operator office (where children were asleep) and in the kitchen, that were accessible to children, they were not in use and no safety covers.

Appliances with heating elements, such as bottle warmers, crock pots, curling irons, and electrical cords were accessible to children. I observed a heated blanket on the couch in the office of the operator, it was not plugged in but the cords were accessible to children.

Children were not cared for in space designated as the caregiving area on the floor plan provided by the operator. Upon arrival, I noticed that children were asleep in the Operators office, this space in not approved to provide care for children.

Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. On the outdoor learning environment, I observed a broken blue children's chair on the children's play space.

An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. Upon arrival, I observed four children asleep in the operators office. There were two children asleep on the floor with no individual mat/cot and two children asleep on the couch.
