Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. There were eight exposed nails on on the wooden fence to the right of the playground.

Potentially hazardous items such as archery equipment, hand and power tools, nails, chemicals, or propane stoves, were not stored in locked areas or with other safeguards, or removed from the premises. There were hand tools such as a pitch fork stored in an unlocked storage building on the playground.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was an aerosol can of sunscreen and bug repellent stored in a unlocked cabinet

Over-the-counter medicines were not in their original containers or administered as authorized in writing by parent, physician or authorized health professional. Two children under two years of age had Motrin on site with parent permission however, the recommended dosage states not for children two and under without doctor's permission. Both bottles of Motrin were not stored in it's original packaging.
