Operator did not wash his/her hands before and after handling food and feeding the children. After provider placed children in the play area, she immediately started preparing their lunch and this requirement was not observed.

Each child's hands were not washed before and after eating. Children were playing in the play area, and when lunch was prepared they were sat at the table an served their meal. Handwashing was not observed.

Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily. there was no policy posted, only the poster was posted.

Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). Providers background check expired in 2021, provider had another letter on file that belonged to someone else with a similar name but the address and birthdate was different.
