A current activity plan was not posted for each group of children for reference. In space 104 there was no activity plan posted.

A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The ABCMS roster has not been updated to include all of the staff members that is employed at the facility.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. There were 10 topical medication permission to administer forms that did not contain all of the required information, such as the amount of the medication to administer and the dates that the medications would expire.

A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. The Prevention of Shaken Baby and Abusive Head Trauma form for (2) children did not contain the children’s date of enrollment.

Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was completed March 2024, and another inspection was not completed until May 2025.
