Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. the gate measures at 45 inches and 47 inches in the back right where the section of the fence is broken.

All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. The older preschool classroom had places on the wall under the window near the tables that had peeling paint. One area looks as if some one was digging in the dry wall.

Bed, cribs, playpens, cots or mats were not placed at least 18" apart or separated by partitions when in use. In space one and two children were laying down cots. That cots were placed less than 18" apart and was measuring at 13 inches.
