Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. During operating hours on July 28, 2024, the provider's son burned a portion of the back yard with a propane weed burner. There were four preschool age children present at the time.

The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The provider did not follow local ordinances with the no open burn in the city limits of Charlotte.

All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. A bottle of Acetaminophen belonging to the provider was located on the kitchen counter.

Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. A aerosol can of Raid was located on the kitchen counter.

Operator did not safely store all combustible materials. A burning candle was located on the kitchen candle.

Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. Documentation for visually checking on an infant less than 12 months of age was not on file since May 8, 2025.

Children were sleeping or napping in a room with a closed door between the child and the supervising staff. A three-month-old infant was placed in another room with a closed door blocking the operator's ability to supervise the infant.

When a child received medical treatment, a copy of the incident report was not mailed to the Division within 7 calendar days after the incident occurred. On February 14, 2025, a one-year-old received medical care for an injury sustained while in care. The operator failed to provide the incident report to NCDCDEE within the required timeframe.

Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. During the visit on February 24, 2025, a gallon of interior wall paint, a 32-ounce container of DAP drywall spackling and a jar of Aquaphor were on the kitchen table located beside the child sized table where children ate lunch. Each container was labeled "Keep out of reach of children." This was corrected during. the visit.

Operator did not provide supervision according to the individual age, needs, and capabilities of each child. On February 14, 2025, a twenty-month-old child sustained an injury, which required medical care, which the operator did not observe and was unable to explain the origin of the injury.

Incident logs were not completed and maintained as required. Two incident reports, dated February 6 and 14, 2025 were not logged on the facility's incident log. This was corrected this during the visit.
