The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. A staff member hired on 8/28/23 and 8/19/24 did not complete the shaken baby head trauma policy prior to caring for children.

In space 3, one child's permission to administer topical ointment form expired 3/31/24. In space 1, one child's prescription medication was not returned after the 7 day course of treatment as labeled on the prescription.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space 2, items were stored in ziploc bags and small plastic bags on a shelf accessible to children.

Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was completed on 9/22/23. The fire inspection was received during today's visit.
