A safe indoor and outdoor environment was not provided for the children. On March 20, 2026, a staff member placed an iPad sitting on the windowsill on a charger in proximity to where a child was napping. The iPad fell from windowsill, hitting a three-year-old child in the left eye causing an injury to the child’s eye.
