The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The centers last shelter-in-place/ lockdown drill was documented for 2/18/25. The center did not complete a drill in 5/2025.

Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. I observed two bottles of hand sanitizer and disinfectant spray on the kitchen counter/ accessible to children.

Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. There were three children present during todays visit and they were not signed in yet when I arrived at 12:00pm. There were five children that were present yesterday that were not signed out on the daily attendance.
