Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Eleven (11) children were being cared for by one (1) staff member, the youngest age being one (1) years old and the oldest being nine (9) years old.

Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu was not current and did not show any substitutions.

Fire drills were not practiced monthly and/or the drill record was incomplete. The fire drill was completed on 4/18/24 but the record of the drill was not recorded and was incomplete.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An electrical outlet in room number two (#2) was uncovered.
