Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Some children enrolled were did not have documentation of daily departure times from the center.

All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. The wall panels on the OLE have three holes in them.

The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Today I observed the following on the OLE: a rusted metal crate, table and two chairs not in good repair, toy containers not in good repair, muddy/ dirty foam mats and rugs (trash).

A valid qualification letter was not on file and available to review at the facility. One (1) therapist that was present during today's visit, was alone with a student doing therapy. The provider did not have a qualification letter on file.

During the walkthrough of the outdoor learning environment, I observed 2 storage bins that were full of molded rotted toys at the bottom of the stairs, along with 1 black storage bin full of water.

A violation was found for which there is no item number. During the walkthrough of the center, I noticed that there was an unlocked door in space #1 near the cozy corner, that leads to space #2. Space #2 leads to the kitchen, and during today's visit, the kitchen door was wide open/ not locked/ accessible to chidlren.
