Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. On the outdoor learning environment, I observed part of the fence, near the parking lot, with a gap at the bottom that was greater than nine inches.

All equipment and furnishings were not in good repair. I observed a play structure on the outdoor learning environment that was not in good repair, it needs to be cleaned due to a buildup of debris.

The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. I observed water in the mop bucket during the walkthrough of the outdoor learning environment.
